A pacing signal processing method, a system and an electrocardiogram (ECG) monitor (100, 200, 300, 400, 500), the method comprises collecting at a high sampling rate the original ECG signal from a body surface, obtaining the parameter and position information of a pacing signal according to the sampling points, and displaying the morphology and/or parameter information of the pacing signal, the pacing signal can be accurately distinguished from interference by means of the repeatability and specificity of the pacing pulse signal morphology and the randomness of interference. The analyzed pacing position and width signals can be used for a smoothing process of the ECG signal to remove the pacing signal from the ECG signal so as to avoid the interference of pacing waveform to the ECG algorithm analysis. Since the smoothing process adopts the pulse width conforming to the pacing signal, an appropriate smoothing width can be selected automatically in accordance with different patient pack-makers and practical signal situations on the body surface so as to retain, to the greatest extent, the effective components of the ECG signal.
