The present invention relates to a shell-and-multi-triple concentric-tube reactor and a heat exchanger and, more specifically, relates to a shell-and-multi-triple concentric-tube reactor and a heat exchanger, capable of optimizing thermal fluidity and the efficiency of heat exchange by providing a new type of reactor and heat exchanger; maximizing the performance of a catalyst by equally distributing reactants and increasing flows of the reactants; and enabling compaction by improving the performance of the reactor. To this end, a shell-side thermal medium flows through a passage formed beside a shell by a baffle, a heat exchange target substance exchanges heat with the outer surface of the passage, and an inflow tube of an internal thermal medium is inserted into the passage in which the heat exchange target substance flows. In the formation of the internal thermal medium, the heat exchange target substance passes through the passage and the internal thermal medium, of which heat is exchanged, is discharged through a discharge tube of the internal thermal medium, which is located inside the inflow tube.
