An apparatus for capturing image information is disclosed. The apparatus includes a waveguide having opposing planar entrance and output surfaces. A diffractive optical element (DOE) is formed across the waveguide. The DOE is configured to couple a portion of the light passing through the waveguide to the waveguide. Light coupled to the waveguide is directed to an exit location on the waveguide via total internal reflection. The apparatus further includes an optical sensor, the optical sensor having an input positioned adjacent thereto for capturing light exiting from the exit location of the waveguide and producing a corresponding output signal. A processor determines the angle and position of the coupled light relative to the input face of the waveguide based on the output signal.
