An aspiration and irrigation system to irrigate the eye and aspirate fluid from the eye during eye surgery, comprising: a phacoemulsification handpiece (112) comprising a body that can be grasped (305) having a distal end and a proximal end; an aspiration conduit (370) configured to transport an aspiration fluid (385) away from a surgical site; an irrigation conduit (340) configured to transport an irrigation fluid to the surgical site; a pump (360) disposed within the handpiece, in which at least a part of the pump establishes an interface with the suction duct and the irrigation duct, so that with the activation of the pump, the fluid from irrigation within the irrigation conduit flows in a direction towards the surgical site, and the aspiration fluid within the aspiration conduit flows in a direction away from the surgical site; a suction valve (390) disposed within the handpiece in the suction duct (370) between the pump (360) and a drain tank (380), the suction valve being configured to control a fluid flow rate of suction inside the suction duct; and a pressure sensor (392) that detects a suction pressure in or along the suction duct; and a controller (405) in communication with the pump (360), the suction valve (390) and the pressure sensor (392), the controller being configured to control the operation of the pump and the suction valve based on information from the pressure sensor.
