The present invention relates to a record paper for a patient on a disaster site, which allows an action to be taken in accordance with the patient′s condition by checking the condition of the patient at a disaster site and classifying the patient according to the degree of severity. The record paper for a patient on a disaster site comprises a record paper, a quick response (QR) tag and a QR sticker. The record paper is configured to be folded to indicate the degree of severity by using color in accordance with basic information on a patient, state information on a patient, a manual classifying a rescued person according to the degree of severity and condition of the patient. A QR tag is formed on one side of the record paper to be cut out and worn on a wrist or ankle of a patient to allow data on the patient to be identified later if the record paper is lost. The QR sticker has a code equal to that of the record paper and the QR tag is formed on one side of the record paper to be cut out and attached to a place requiring the sticker. The condition of a patient is recorded on the record paper with instructions to take action in accordance with the condition of the patient by checking the condition of the patient at a disaster site and classifying the condition of the patient according to the degree of severity. Moreover, the patient′s degree of severity is indicated by using color to allow appropriate action to be easily taken, and it is possible to easily change indications between patient ratings when the patient′s condition changes. Moreover, the record paper where the condition of the patient occurring on a disaster site is recorded is photographed to be applied via an internet network or a mobile communication network, and an image on the applied record paper is computerized through a specific program such that the condition of the patient becomes data, thereby having an effect to easily check the record paper in a hospital or the like even if the record paper is lost.;COPYRIGHT KIPO 2018
