In the present invention, provided are a prescription preparation management apparatus, capable of calculating a preparation price based on a preparation price as prescribed by a doctor and a preparation price for alternative preparation, and providing the preparation prices to a user, a prescription preparation management system using the same, and a method thereof. The prescription preparation management system comprises: a user terminal which selects a preparation price of one pharmacy from preparation price information for each pharmacy for prescription details and preparation price information for each pharmacy for alternative preparation from a management server; a doctor terminal which generates a prescription for each user, transmits the prescription to the management server, and provides an approval or disapproval command for a use approval request of a drug capable of being alternatively prescribed from the management server; and the management server which determines whether a drug capable of being substituted to be prepared exists among drugs of relevant prescription information based on the prescription information for each user, outputs at least one of a preparation price for each pharmacy for prescription details and a preparation price for each pharmacy for alternative preparation, transmits the output preparation price for each pharmacy with pharmacy information to a relevant user terminal, and, as a user selects one preparation price and a pharmacy, notifies a relevant pharmacy of the user′s selection.
