The present invention relates to a pillow integrated with a backrest having backbone and shoulder protection for a reflux esophagitis patient, comprising: a backrest part formed to be inclined at a predetermined angle so as to support a back region of the body; a shoulder support part formed to be inclined at the same angle as the backrest part to support a shoulder region of the body; and a headrest part formed in a shape of a waveform having different heights so as to support a head region of the body. The reflux esophagitis patient can lie down at the predetermined angle so that a gastric acid reflux can be prevented and a cervical vertebrae can be maintained at a higher position than a chest region of a user. As a result, the gastric acid reflux can be prevented by maintaining the cervical vertebrae at the higher position than the chest region while not only lying on the back but also lying on the side.
