The present invention relates to a scarf osteotomy device to fix and maintain distal and proximal bone pieces. According to the present invention, scarf osteotomy device comprises: a first member coupled to the proximal bone piece, including a first surface and a second surface forming a predetermined angle with respect to the first surface while connected to one side of the first surface, having a first osteotomy guide gap on the other side of the first surface in the direction intersecting a first end part, and having a cylindrical part extended in one direction; a second member connected to the distal bone piece, including a guide reception part to receive and guide the cylindrical part, a third surface to be spaced apart from the first surface to form a second osteotomy guide gap, and a fourth surface to be spaced apart from the second surface to form a third osteotomy guide gap, and connected to relatively move to the first member in the extension direction of the cylindrical part to allow movement of the distal bone piece; and a third member coupled to the first member and including a pressing member to press the second member in order to move the distal bone piece in the one direction when the proximal and distal bone pieces are in an osteotomy state with each other. To prevent recession between the proximal and the distal bone pieces, a recession prevention plate is inserted into the second osteotomy guide gap of the second member.
