PROBLEM TO BE SOLVED: To easily preserve a physical document or the like as electronic data, attach required information, and prevent spoofing and alteration.SOLUTION: A web page generation part 106 generates a web page according to a kind of a format accepted by an information terminal device 200, and a transmission/reception part 101 transmits the web page to the information terminal device 200. The transmission/reception part 101 receives, from the information terminal device 200, image data in which an object is imaged by instructing an imaging region contained in the web page, own writing data generated from own writing accepted in an own writing region contained in the web page, and attached data which is accepted in an attached data input region contained in the web page.
