The invention describes a device (1) for decompressive craniotomy, which comprises: a first plate (2) with a first end (21) configured to be attached to a bone flap (CO); and a telescopic element (3) that has a base (31) configured to be attached to the cranium (C) and an upper end (32) configured to be attached to a second end of the plate (22). According to the invention, the base (31) of the element (3) comprises: a first ball joint configured such that, when the base (31) is attached to the cranium (C), the extendible telescopic element (3) can be inclined in relation to the cranium (C); and a locking means configured to set an inclination of the element (3), the upper end (32) of the element (3) comprising a second ball joint configured such that, when the upper end (32) is attached to the plate (2), the plate (2) can be inclined in relation to the element (3).
