The present invention relates to the field of medical treatment, and discloses a method for determining target positions using a radiotherapy apparatus, a device, and a radiotherapy apparatus, so as to accurately detect position changes of a tumor. The radiotherapy apparatus comprises a ray source (1) and a detector (3) for receiving rays emitted by the ray source (1). The method for determining target positions using the radiotherapy apparatus comprises: the ray source (1) located in a first position (11) emitting a radiation beam (S101); the detector (3) receiving the radiation beam emitted by the ray source (1) from the first position (11) and generating, according to the received radiation beam, first image data of a target (S102); the ray source (1) moving to a second position (12) and emitting a radiation beam, wherein an interval at which the ray source (1) moves from the first position to the second position is a positive integer multiple of a preset breathing cycle of a patient (S103); the detector (3) receiving the radiation beam emitted by the ray source (1) from the second position (12) and generating, according to the received radiation beam, second image data of the target (S104); and determining, according to the first image data and the second image data, position information of the target (S105).
