PROBLEM TO BE SOLVED: To reduce applicant's troublesomeness for inputting set specific information, and to reduce a labor to manage the specific information at a system side in the case of opening an account or issuing a card for an employee of a specific enterprise or the like.SOLUTION: A service use application registration system includes a portable terminal device 10 having: a display part 14 for displaying an input screen for inputting data necessary for application to use of a service; an input screen control part 15 for controlling the input screen; a camera 11 for inputting a two-dimensional code image (code image for identification) and certificate data necessary for the use of the service; a two-dimensional code recognition part 12 for recognizing specific information indicated by the two-dimensional code image input from the camera 11; an application data input part 17 for inputting the application data of the use of the service; and a transmission part 19 for transmitting the recognized specific information, the input certificate data and the application data.SELECTED DRAWING: Figure 2
