An objective of the present invention is to directly connect a precast concrete bracket to a lining of a tunnel by a bolt or the like. According to an embodiment of the present invention, a tunnel lining form apparatus with an insert to be buried therein to couple a precast concrete bracket for supporting an air duct slab comprises: a mold having an insert insertion hole into which an insert is inserted; a reinforcing member allowing the insert insertion hole to be correspondingly welded and coupled with respect to the mold, and having an insert insertion reinforcing hole being formed in a thickness direction and reinforcing the thickness of the insert insertion hole, and a first connection pin penetration hole formed therethrough in a direction perpendicular to the thickness direction; a cylindrical bolt having a cylindrical body unit with a second connection pin penetration hole formed in a direction perpendicular to the direction of the insert insertion reinforcing hole and locked and coupled to the reinforcing member by a connection pin, a bolt unit extended from one end of the cylindrical body unit, and a head unit arranged on the upper end of the cylindrical body unit and used as a handle when released; and an insert having a bracket fixing part screw-coupled to the bolt unit of the cylindrical bolt to be arranged on the back surface of the mold and coupled to a bracket for supporting an air duct slab, and a reinforcing bar unit extended from the bracket fixing part.;COPYRIGHT KIPO 2020
