The case of a 50 year old male ironworker foreman who died from injuries receivedin a 24 foot fall from the forks of a forklift was reported. He was employed by a steel erection contractor that employed 40 workers. The employer has been contracted to erect the skeleton steel and lay the corrugated metal subflooring for a warehouse under construction. The victim was supervising six workers and had moved a forklift under the area where flooring was being moved into place at the second story level. He instructed a worker to operate the forklift and raise him to the work area. Once at a height of about 24 feet off the ground, the victim attempted to step from one fork of the fork lift to the opposite fork, slipped and fell to the ground, striking his head and chest. He was pronounced dead at the scene. The cause of death was blunt force trauma to the head and chest. Recommendations include that workers adhere to general safety practices for elevating personnel, that existing written safety programs be reviewed, that workplace safety inspections be carried out, and that workers be encouraged to actively participate in workplace safety.
