The report concerned the death of a 21 year old male electrician who was workingfor an electrical contractor. On the day of the accident the victim and his apprentice were working at a locomotive repair building repairing electrical equipment. The men had climbed onto the roof of the facility to gain access to an electric motor and wiring at one of the ventilators. The roof had corrugated fiberglass panels interspersed among corrugated metal panels; due to weathering, the fiberglass panels were barely distinguishable. As the victim moved around the ventilator, he accidentally stepped on a corrugated fiberglass roof panel, fell through the panel, and landed on the concrete floor 60 feet below. He was pronounced brain dead 43 hours later due to blunt force head trauma. Recommendations include evaluating current safety programs and incorporating specific training procedures which stress recognition and avoidance of hazards at the worksite, and evaluating and identifying hazards and either prohibiting access to hazardous areas or eliminating the hazard before access is permitted.
