This report explains two explosions that took place on January 7, 1998, at an211u001eexplosives manufacturing facility owned by Sierra Chemical Company, located in 211u001eMustang, Nevada. Four workers were killed and six were injured. Safety issues 211u001ecovered in the report include process safety management, process hazard analysis, 211u001etraining, language barriers, operating procedures, building siting, and employee 211u001eparticipation. Recommendations concerning these issues were made to Sierra 211u001eChemical Company and other companies manufacturing explosives, the Institute of 211u001eMakers of Explosives, the Nevada Occupational Safety and Health Enforcement 211u001eSection, and the Department of Defense.
