摘要

This study was designed to understand the “landscape” of hospital incident reporting systems and to examine the use of health information technology to improve reporting, data analysis and learning from errors in health care. The field agrees that standardized collection of patient safety data will help to identify causes, contributing factors and the effects of adverse events in order to learn from both medical errors and system-related failures.
