Grant Application Forms and Instructions for Demonstration Projects of Integrated Service Delivery Systems for Human Service Programs

This document presents an article from The Federal Register which announces the availability of funds and requests State grant applications under the Department of Health and Human Services Demonstration Program. The purpose of the program, to improve the management and delivery of human services, is defined, and program goals are listed. Additionally, the nature and scope of the projects to be funded are examined, applicant eligibility is described, and selection criteria are noted. Application forms are included, and instructions for completing these forms are supplied.
