Canine kidneys preserved by hypothermic perfusion were subsequently autotransplanted, and contralateral nephrectomy was performed 7 days later. Dimethylsulfoxide (DMSO) was added to the perfusate of experimental kidneys, up to a concentration of 10% and removed in stepwise fashion to a concentration of 2.5% before autotransplantation. Renal functions, indicated that only reabsorption of glucose, on the day of perfusion, was significantly less in DMSO-perfused kidneys than in controls. After contralateral nephrectomy, serial serum creatinine and urea levels attested to good renal function in both groups. In a second series of experiments, contralateral nephrectomy was performed at the time of autotransplantation, and renal function was assessec by the degree of azotemia seen during the next 4 weeks, these experiments also tested the value of osmotic diuresis immediately after autotransplantation. It was concluded that 10% DMSO, a concentration likely to be cryoprotective, produces little renal damage. However, analysis of DMSO in renal tissue revealed that the equilibration of the compound between perfusate and the medulla was incomplete, and this situation may be incompatible with cryophylaxis of the organ.
