Does vitrectomy increase the risk of glaucoma?

The article by Lalezary et al seeks to address an important question about whether vitrectomy increases the risk of developing glaucoma. Two prior publications by Chang and Luk et al suggest that vitrectomy increases the risk of developing glaucoma, particularly in pseudophakic/aphakic eyes. The study by Lalezary et al, recently published in the April issue of RETINA, and one previous study by Yu et al show no increased risk of intraocular pressure (IOP) elevation after vitrectomy. This raises the question about why the current study disagrees with the findings from the two previous studies. All four studies are retrospective case series, and differences between the study designs may help to explain why the conclusions differ. The article by Chang1 started with a group of 1,156 eyes. Many eyes were excluded because they were not followed for the minimum duration of 6 months and because of other exclusion factors, leaving 453 eyes. Sixty-eight of these eyes developed glaucoma, suspected glaucoma, or had preexisting glaucoma.
