This partly explains why lumbar segmen-tal arterial injury from traditional lumbar TFESIs could cause paraplegia. This degree of variability suggests that the actual foramen traversed by the ARM will remain unknown when a TFESI is performed. Ideally, a practitioner should know the ARM location given the catastrophic nature of paraplegia and the elective nature of a thoracolumbar TFESI. Pre-procedure localization of the ARM would enable a practitioner to individualize, rather than guess risk. One could not practically offer spinal angiography or non-invasive cross sectional imaging in anticipation of every TFESI.
