Workers for a metal fabrication company were forced to improvise the lift that led to a fatal accident at London's Francis Crick Institute because the method statement was so unclear, an HSE inspector has said. Feltham based CMF, which was contracted to install six staircases at the building, was fined 185,000 pounds over the death of subcontractor Richard Laco, 31, who was crushed when a pre-fab-ricated metal landing weighing 1.1 tonnes slipped from the sling of an electric hoist.
