The authors report a case of intratesticular adrenocortical adenoma, 3 cm in diameter diagnosed in a 35 year-old man after right orchidectomy. He denied any past medical problems, surgery or prescription drugs. Physical examination revealed a hard, painless nodule of the lower pole of the testis. He presented no evidence of gynaecomastia or endocrine dysfunction. Serum tumour markers (HCG, AFP, LDH) were within normal limits. Ultrasound revealed a well-circumscribed, complex intraparenchymal mass with a largely hypoechoic appearance. Abdominal CT scan showed the presence of the adrenal glands in the normal position with a normal appearance The preoperative diagnosis of benign testicular tumour is difficult. Intraoperative frozen section histological examination can be performed to avoid unnecessary orchidectomy.
