Activated vitamin D sterols are used to manage secondary hyperparathyroidism. As kidney function declines, the production of activated vitamin D (1,25-dihydroxyvitamin D) and serum calcium concentrations drop, phosphate retention ensues, and the aggregate culminates in secondary hyperparathyroidism.Activated vitamin D directly and indirectly (by increasing serum calcium) suppresses the synthesis and secretion of parathyroid hormone (PTH), and high concentrations of PTH are indicators of osteitis fibrosa, a bone disorder that accompanies renal failure.
