Home orthostatic training in vasovagal syncope modifies autonomic tone: results of a randomized, placebo-controlled pilot study.

The paper by Tan et al. deals with reflex syncope (neurally mediated syncope): a terminology adopted by the European Society of Cardiology. The authors conclude: 'This was the first placebo-controlled study in orthostatic training which has: (i) demonstrated that such a study is indeed feasible; (ii) an enhancement in overall autonomic tone is observed with home orthostatic training (HOT), in tandem with a non-significant trend in symptom improvement; and (iii) a larger adequately powered randomized controlled trial of tilt training is now needed.'
