In a 60-year-old man with vasospastic angina, a transient anteroseptal myocardial perfusion defect was demonstrated on SPECT after hyperventilation. The occurrence of spasm of the left anterior descending coronary artery was confirmed later by coronary angiographicstudy, while the perfusing area of the artery was compatible with the location of the defect. Myocardial scintigraphy after hyperventilation may provide invaluable information for the diagnosis of vasospastic angina. The technique may be applied to patients with equivocal electrocardiographic changes for the occurrence of transient myocardial ischemia.
