A 38-year old man had persistent hyperparathyroidism despite two previous (neck) parathyroidectomies. Subsequently, ectopic parathyroid gland activity in the mediastinum was demonstrated on a Tc-99m sestamibi scan. The tumor localization in the mediastinum was confirmed by placing a "cold" marker over the focal parathyroid activity, and then by performing a gated blood pool study, which outlined the right heart, pulmonary artery, and the left heart and ascending aorta. At surgery, two small ectopic parathyroid glands were found in the aortopulmonary window, where the marker was placed. These represented pathology proven parathyroid adenomas.
