Psoriatic Dactylitis: Bone Scintigraphic Appearances

A 33-year-old woman had isolated painful dactylitis of the left middle finger of 9 months' duration. Chronic indolent infection was suspected. Radiographs demonstrated diffuse soft tissue thickening only. The result of a subsequent bone scan was grossly abnormal. Rheumatologic assessment revealed psoriatic nail dystrophy in other digits, scaling of the scalp and elbow, and minimal restriction of cervical spine movement only. Psoriatic dactylitis was diagnosed, to which the patient improved after corticosteroid injections of the flexor tendon sheath and involved joints.
