While being evaluated for polycystic ovarian syndrome by ultrasound scan, a 24-year-old gravida 0, para 0 woman was found to have a pelvic mass. Her past medical history was significant for obesity, amenorrhea, and marked hirsutism. She was asymptomatic, the findings from her physical examination were noncontributory, and her CA 125 level was not elevated. Computed tomography scan revealed the mass to be cystic, measuring 10.2 cm in diameter, and arising from her right ovary. The patient underwent a laparoscopic right ovarian cystotomy/cys-tectomy.
