Morphine augmentation has recently been advocated to expedite iminodiacetic biliary imaging (1,2). In this case report, false-negative imaging occurred despite surgically proved cystic duct occlusion owing to an impacted, small cystic duct stone. This raises the possibility that morphine-induced spasm of the sphincter of Oddi may raise biliary pressure sufficiently for gallbladder isotope filling, despite an impacted cystic duct stone and acute cholecystitis.
