Postpartum hepatic necrosis in a patient with the syndrome of hamolysis, elevated liver enzymes, and low platelet count was diagnosed by Tc-99m SC liver-spleen imaging. Subsequent CT and ultrasonographic studies confirmed the nuclear medicine findings and suggested partial hepatic regeneration. Despite aggressive nonsurgical supportive care, the patient died. An autopsy confirmed focal massive hepatic necrosis.
