There is controversy about whether trapeziectomy should be combined with ligament reconstruction and tendon interposition (LRTI). When LRTI is done using the flexor carpi radialis (FCR) tendon, there is debate about whether a split tendon or the whole tendon graft is taken. We describe a technique for harvesting half of the FCR tendon for use in LRTI following trapeziectomy.A trapeziectomy is carried out through a dorsoradial incision. The distal portion of the FCR tendon is preserved. A 1 cm transverse skin incision is made over the FCR tendon, approximately 10 cm proximal to the flexor wrist crease. FCR is identified and the sheath opened. A 20-gauge wire (wire A) is doubled such that a tight loop is created at the mid-point of the wire.
