Urinary macromolecules appear to provide the major source of inhibitory activity and provide an important protective role in calcium oxalate urinary lithiasis. However, uncertainty still exists concerning the question of which of these substances is the principal inhibitor(s). Gene therapy offers exciting avenues for further developments in medical management, but there is still an important place for well designed clinical trials if medical management is to be improved.
