Diffuse myocardial uptake of pyrophosphate has been reported in acute myocardial infarction, unstable angina, cardiomyopathy, myocarditis and amyloidosis. Persistent blood pool activity has been reported also to give the appearance of diffuse myocardial uptake. This is the first reported case of diffuse uptake using Tc-99m MDP during screening for evidence of child abuse. The localization of Tc-99m MDP was due to diffuse myocardial ischemia secondary to poor cardiac output following cardiac arrest.
