Dissociated responsiveness of a growth hormone- and thyrotropin-secreting pituitary adenoma to octreotide-long-acting release therapy: The intriguing case of Mister B.

Mr B., a 61 -yr-old man with history of chronic obstructive pulmonary disease, was referred to our outpatient clinic, complaining of subclinical symptoms of thyrotoxico-sis, i.e. sinus tachycardia (100 pulses/min) and increased pulse pressure (70 mmHg). Increased serum free T3 (FT3) (14.0 pmol/l), free T4 (FT4) (51.4 pmol/l), and TSH (2.0 mU/l) were found and, consequently, a pituitary function evaluation was performed. Serum GH (8.6 ng/ml) and IGF-I (537 ng/ml) levels were elevated and GH was not suppressed during standard oral glucose tolerance test (Table 1).
