Patient presentation. A 41-year-old man with unknown heart surgery at 14 years of age was admitted with exertional chest heaviness, radiating to the left arm, associated with dyspnea and nausea. The findings of the physical examination were unremarkable, with no murmur. Serial creatine phosphokinase and troponin levels and the electrocardiogram were all normal. The patient's medical history is significant for an asymptomatic heart murmur noted at 14 years of age. Cardiac catheterization and heart surgery were performed. However, his clinicians were unable to locate any information about the diagnosis or procedure that had been performed.
