A 55-year-old woman with diabetes had an ulcer in the soft tissues under the fifth metatarsal, and a request was made to rute out osteomyelitis. Discussion with the patient revealed marked left groin pain of recent onset. A combined three-phase bone/in-111 leukocyte study confirmed osteomyelitis of the fifth metatarsal. In addition, there was intense uptake in the lymph nodes of the left groin region representing lymphatic spread of infection. After 6 weeks of antibiotic therapy, the patient became asymptomatic. A repeat in-111 leukocyte study at that time showed no uptake in the groin.
