Recent studies have shown that preferential diminution of renal cortical perfusion is a common link between a variety of insults that result in acute renal failure. The effect of intravenous tris (hydroxymethyl) amino methane (THAM) infusion on renal cortical ischemia and oliguria during sustained hemorrhagic hypotension was studied in six anesthetized dogs. Between 180 mM and 360 mM THAM infusion, a remarkable restoration of cortical perfusion and urine flow occurred. Six parallel studies using NaHCO3infusions failed to produce similar results, despite an identical effect on arterialpH. THAM infusion can reverse the oliguria and renal cortical ischemia of sustained hemorrhagic shock by medianism(s) different from its acid-base buffering effect.
