Results of a chest radiograph taken in a 60-year-old woman with evidence of adrenal hyperfunction were within normal limits. In an effort to localize abnormal adrenal tissue, an In-111 octreotide study was done. Marked uptake was seen in both lung fields, on images obtained 6, 24, and 48 hours after administration. Lung biopsy showed changes that were consistent with a hypersensitivity reaction. Stains for fungi showed spherical bodies 7 to 12 mu;m in diameter. There was clearing of an area surrounding each fungal capsule, suggesting an external coat. The patient's concentration of In-111 octreotide in the lungs was likely related to a hypersensitive reaction to the fungi.
