In a 68-year-old woman with autoimmune hemolytic anemia, there was considerable (and diffuse) splenic accumulation of Tc-99m MDP. Because diphosphonates have an affinity for iron compounds, the spleen was examined for hemosiderin. A considerable amount of hemosiderin uptake could be demonstrated throughout the spleen. This may be another example of extraosseous uptake of a Tc-99m labeled diphosphonate related to the presence of iron compounds in the tissue.
