Good medical practice demands well-kept records. In general, slipshod practice goes hand in hand with slipshod records. Good records point the path to better diagnosis and offer a reminder of errors or inadequacies in management and therapy. The authors have set an example which might well be emulated by others, no matter what the field of practice.
