Dear Sir, The non-dominant right hand of a previously fit and healthy 25 year-old male ground worker was trapped for several seconds under a 1 tonne bucket of hardcore on 13 October 2006. The tissues of the first web space, thenar and hyopthenar eminances were extruded through the skin (Figs 1 and 2), and there were fracture-dislocations of the 1st, 2nd and 3rd carpome-tacarpal joints, together with disruption of the scapho-trapeziotrapezoid joint (Fig 3). He was of average build and denied taking medication or recreational drugs.
