Multiple organ concentration of Tc-99m MDP in a 60- year-old man who had primary hyperparathyroidism and persistent, severe hypercalcemia is reported. This patient was hospitalized for the evaluation of severe, generalized bone pain. A bone scan performed with Tc-99m MDP did not reveal any focal bony abnormality. Intense and diffuse accumulation of Tc-99m MDP, however, was seen in the myocardium, lungs, diaphragm, stomach, spleen, and kidneys, refecting soft tissue calcinosis as a result of prolonged hypercalcemia.
