An 80-year-old farmer with unresectable hepatocellular carcinoma was referred to the authorsrsquo; department for evaluation of the somatostatin receptor status of his tumor. After intravenous administration of In-111 gadoliniumndash;diethylenetriame pentaacetic acid (DTPA) octreotide (OctreoScanreg;, 5 mCi), the patient was imaged 24 hours and 48 hours later. Besides the presence of somatostatin receptors in his primary tumor and 1 intraabdominal metastatic site, diffuse pulmonary and heart uptake was observed. A review of the patient's history revealed that he had pulmonary fibrosis, resulting from hypersensitivity pneumonitis, secondary to his occupational exposure to chicken-derived antigens. The activated lymphocytic pulmonary and heart infiltration is the most likely explanation for the observed In-111 DTPA octreotide uptake.
