NASA's Columbia space shuttle 'successfully' launched for the 28th time on 16 January 2003. Or did it? On an otherwise normal Saturday morning on 1 February 2003, Columbia broke up as it returned to earth, killing all seven crew on board. The Columbia disaster was the catalyst for much-needed change to NASA as an organisation and structure, as well as its values and culture. That day also irreversibly changed Harmony Myers, Director of NASA Safety Centre.
