The left-sided gallbladder is a rare anatomical variant. Given the highest risk of association with other variations of the biliary tract, surgical management of such condition requires particular attention. We report a case of an incidental intraoperative discovery of a true left-sided gallbladder in a 30-week pregnant woman admitted to our department for acute cholecystitis. Severe inflammation and the narrow working space (gravid uterus) associated with the presence of this rare anatomic variant made the laparoscopic cholecystectomy challenging, in our case.
