Cataract surgery is the most performed operation worldwide. Being a cataract surgeon requires years of training, technical skill, and lifelong learning. Cataract surgery usually delivers exceptional outcomes, but inherent within the procedure and the personality of surgeons themselves lies some perfectionism. Ophthalmologists speaking at meetings highlight minimal complications and extraordinary outcomes. This creates the perfect trap-a false concept that cataract surgery is a perfect operation. Cataract surgeons know that in the real world this is not so. We know to expect the unexpected when operating-patients cough, the phacoemulsification machine malfunctions, the posterior capsule is ruptured, the trainee struggles, or even earthquakes occur. 1Most cataract surgeons have moments of anxiety, doubt, or are upset by bad outcomes. These outcomes challenge why and how we perform cataract surgery.
