Dear Colleague: Journal of Palliative Medicine is facing a generational change. The past 25 years have been about successfully establishing the evidence base for the practice of this new subspecialty. Journal of Palliative Medicine has published the work that has underpinned this success. Palliative medicine is now an established clinical and research subspecialty in high demand the world over. The inevitable product of science is the discovery of pressing new questions. The next generation must grapple with how best to deploy the subspecialist workforce in the face of clinical demand exceeding supply. What should be the purview of subspecialists and what should be shouldered by others? What of our existing practice should be abandoned in favor of better treatments? What improvements in patient care are achieved when palliative medicine is combined with other subspecialist branches of medicine early in the course of serious illness? How are contemporary audiences best reached? New leadership is essential if Journal of Palliative Medicine is to remain at the leading edge of palliative medicine.
