A 27-year-old-female with a history of cervical radiculopathy due to degenerative cervical disc disease (C4/C5, C5/C6) was brought to the operating room of Cayuga Medical Center for Anterior Cervical Discectomy and Fusion procedure of cervical vertebra C4-C6. Her preoperative blood work including hemo-gram and basic metabolic panel was unremarkable. While in the operating table, she was administered succinylcholine 200 mg for induction of anesthesia. While positioning her and preparing her for preoperative monitoring by somatosensory evoked potentials and motor evoked potentials, there was difficulty in obtaining any motor or sensory responses. She was given several troubleshootings and the Train-of-Four, but was still unable to obtain any response or movements. After 1 h, trouble shooting and Train-of-four were repeated but no response was achieved.
