In September 2006, a crane collapsed on a housing development in south-west London, killing the operator and a member of the public. The subsequent investigation found that the crane's turret and jib separated from the mast when 24 bolts failed due to metal fatigue. It was more than five and a half years before the inquest into the deaths of the two men started. It was a further four years -nearly ten years after the incident - before Falcon Crane Hire, the company that employed the crane operator, was fined 750,000 pounds.
